People often wait until they are seriously ill before thinking or talking about end of life. At Greenwood Health we feel it is very important to have these discussions with your whānau/family while you are well.

Our Practice Nurse Megan Wealleans offers an Advance Care Planning (ACP) service, which is available to patients at any point in their lives.

Often it is at one of life’s crossroads (making a will for the first time, children leaving home or a life-altering medical diagnosis, for example) that we pause and evaluate how we have lived our lives and what aspirations we have for the future.

ACP is about sharing that understanding with family, and recording and formalising thoughts and wishes about future health care. As an experienced Practice Nurse, Megan is well placed to provide an in-depth understanding of your current and future health treatment and care options.

Please ring and ask to speak to Megan about this service.

On nights, weekends and public holidays a Motueka GP or Nurse Practitioner will be on call to provide After Hours Urgent Care for registered patients and visitors.

The usual daytime telephone number for Greenwood Health will be answered by a trained Nurse who will discuss your Urgent Healthcare need and organise the best course of action, which may include contacting the on call GP.

If you require emergency assistance then please dial 111 and ask for the ambulance.

Heart disease is the leading cause of mortality in New Zealand. Many heart related deaths are premature and preventable. Regular heart checks are often performed by our GPs or Nurses based on information available in registered patient records (like recent blood test results or blood pressure recording), the purpose of this is to establish potential issues early and provide advice that can enable lifestyle choices or treatment options and lower the risk of heart attack or disease. Contact and ask to speak to one of our Nurses if you wish to know more.

If you need to be seen at home, we can arrange for a doctor or nurse to visit you. Please ring to discuss your situation.
